Job Description

Based in Racine, Wisconsin, Lily Home and Hospice is seeking a Hospice Liaison with RN preference to join its on-site team. This role focuses on building durable referral networks and educating healthcare professionals and families about hospice services, with a clear emphasis on driving growth within the community.

Responsibilities

Develop and maintain strong referral relationships with hospitals, physicians, SNFs, ALFs, and community partners
Educate healthcare professionals and families about hospice services
Serve as a trusted resource and advocate for patients transitioning to hospice care
Collaborate closely with clinical and leadership teams to support growth initiatives

Requirements

RN preferred
Experience in hospice, home health, healthcare sales, or business development strongly preferred
Excellent communication and relationship-building skills
Compassionate, self-motivated, and goal-oriented
